New Patient Registration
If you are a resident within our catchment area you may request to register with the practice.

You will need to complete a form GMS1 and it is helpful to have your NHS number and the name and address of your previous GP. You will also be asked to fill in a questionnaire to complete for information pending the arrival of your records from your last GP. If you have any specific need especially when we communicate with you please let us know at registration.
Please print off the registration forms from the website, fill them out and email them to us or drop them through our letter box. Please also remember to supply photo id as proof of address, such as a passport of driving licence. The surgery brochure is also shown below for you to read & print if you require.
COVID UPDATES TO NEW REGISTRATONS
Due to the current COVID restrictions, please can you email a copy of your completed forms to email@example.com or post your forms to either of our surgeries.
New baby registrations - Due to the current situation we can register your baby without a birth certificate. Once restrictions are lifted and you are able to register your baby at the register office, please bring in a copy of the birth certificate into either surgery so that we can take a copy of this.

If you wish to change your preferred GP, please request this through E-consult or in writting but this may not always be possible if your preferred GP already has a large patient list.
